Is this a subscription?
No. Every option is a one-time purchase with a perpetual license. You keep the version you bought, with free updates inside its major version.
What's the difference between the three?
The Premiere / After Effects extension runs inside Adobe and grades clips on your timeline. The Mac app is a standalone studio for quick looks and batch passes. The Ultimate Bundle is both, on one license, for $69 — and it's the only way to get the DaVinci Resolve plugin, in beta.
Do I pay tax on top?

What do I need to run it?
macOS 13 Ventura or later, Apple Silicon. The extension also needs Premiere Pro / After Effects 2023 or later. The bundle's DaVinci Resolve plugin (beta) works in the free edition of Resolve.
